Medication and Immunization Information

Use these forms to authorize medication to be administered to your child, and find out which immunizations are required to enter each grade level.Parents may also contact the school clinic for copies of the forms. Read more.

District Hearing/Vision Screening

Vision and hearing screening is conducted for students in their schools according to the Colorado Department of Education guidelines. These guidelines indicate that students in kindergarten, first, second, third, fifth, seventh and nineth grades, and exceptional needs students receive vision and hearing screening. Because of the important connection between a student's academic achievement and their vision and hearing, parents are encouraged to observe their student and seek a medical evaluation for changing or signficant concerns. The school clinic aide may be contacted for information regarding health and community resources available to parents.

Section 504 of the Rehabilitation Act of 1973 is Congress’s directive to schools receiving any federal funding to eliminate discrimination based on disability from all aspects of school operation. Read more.

School Medicaid Program

The district School Medicaid Program serves to enhance or expand health and related services for all students. This program provides the opportunity for parents to meet with program staff and enroll eligible students in CHP+ or Medicaid insurance. Jeffco Homebound Instruction Program

The Homebound Instruction Program (HIP) is designed to serve students who are unable to attend school for an extended time because of a physical and/or emotional health concern.

Healthy Schools

Jeffco Schools has received a "Healthy Schools Colorado" grant from the Rocky Mountain Center for Health Promotion and Education (RMC). The grant allows the district to support efforts related to school health and wellness using the coordinated School Health Model developed by the Centers for Disease Control and Prevention.
