The Jeffco Department of Health Services maintains the goal of a holistic approach while providing health support to all regular and exceptional needs students in Jefferson County Public Schools.
Use these forms to authorize medication to be administered to your child, and find out which immunizations are required to enter each grade level. Parents may also contact the school clinic for copies of the forms. Read more.
Asthma Care Plan
Students diagnosed with asthma and require an inhaler or medication at school will need to have their health care provider and parents complete the Colorado Asthma Care Plan and return to the school clinic aide. Parents of student with asthma or health concerns are also encouraged to contact their school clinic aide to follow up regarding care and support at school.
Students with Food Allergies
The Jefferson County Board of Education recognizes that many students are being diagnosed with potentially life-threatening food allergies. To address this issue and meet state law requirements concerning the management of food allergies and anaphylaxis among students, the board has established Policy JLCDA, which specifies details regarding a health care plan, reasonable accommodations, access to emergency medications and staff training.
Parents, the school administrator and the school nurse will develop a health plan using the Student Allergy and Anaphylaxis Health Form.
District Hearing/Vision Screening
Vision and hearing screening is conducted for students in their schools according to the Colorado Department of Education guidelines. These guidelines indicate that students in kindergarten, first, second, third, fifth, seventh and ninth grades, and exceptional needs students receive vision and hearing screening. Because of the important connection between a student's academic achievement and their vision and hearing, parents are encouraged to observe their student and seek a medical evaluation for changing or significant concerns. Section 504 of the Rehabilitation Act of 1973 is Congress’s directive to schools receiving any federal funding to eliminate discrimination based on disability from all aspects of school operation. Read more.
Jeffco Schools has received a "Healthy Schools Colorado" grant from the Rocky Mountain Center for Health Promotion and Education (RMC). The grant allows the district to support efforts related to school health and wellness using the coordinated School Health Model developed by the Centers for Disease Control and Prevention. Accident and Dental Insurance
Our students’ safety and well-being is our greatest concern and we want them to have medical care in case of an accident. We encourage parents and guardians to enroll their children in accident and medical insurance programs. There are several low cost plans available including K & K Insurance Group. Check with an insurance company for additional plans.
The State of Colorado offers Child Health Plan Plus (CHP+), which is for families that earn too much to qualify for Medicaid, but cannot afford private health insurance. The State of Colorado Website provides additional information about medical insurance programs.
School Medicaid Program
The district School Medicaid Program serves to enhance or expand health and related services for all students. This program provides the opportunity for parents to meet with program staff and enroll eligible students in CHP+ or Medicaid insurance.
