Required Documentation Checklist

Completing and submitting the online application and required documents does not constitute enrollment in Jeffco's 21st Century Virtual Academy. The Virtual Academy is unable to determine a student's eligibility for enrollment until all documentation is received.

Within five days of completing your online application, fax or mail the following required documents to the Virtual Academy:

Completed Self Assessment
Completed Enrollment Acceptance Form
Completed Student Information Form
Signed Conduct Code Review Form
Signed Student Use of the Internet Agreement
Completed Family Economic Survey
Official school transcript
Copy of student's official birth certificate or passport1
Completed Statement of Residency1
Copy of up-to-date immunization records1
Signed Parent Portal Access Form (if applicable)
Copy of most recent IEP, ALP or 504 accommodations (if applicable)
