School to Work Alliance Program (SWAP)
The purpose of Transition Services is to assist young adults with disabilities in making the transition from school to employment. Disabilities can include, but are not limited to physical, emotional and learning disabilities. SWAP provides opportunities for young adults to practice and improve workplace skills, consider their career interests and obtain real-world work experience that will lead to competitive, integrated employment in today's high-demand industries.
SWAP Services
While in high school, SWAP services may include:
- Career awareness, exploration and preparation
- Work readiness training
- Self-advocacy training
- Guidance and counseling around career training and education
- Work experience
After exiting high school, SWAP services may include:
- Job-seeking skills training
- Job-search activities
- Job placement
- Job coaching
- Job retention skills
General services may include:
- Pre-employment transition services
- Employment-related instruction
- Case management
- One year of follow-up
- Year-round community-based service
- Summer programs:
- SWAP provides opportunities for youth to explore careers and training and college programs during the SWAP Summer Safari.
- Summer Safari is a partnership with community centers and industry partners to provide career exploration and job skills.
SWAP is a service provider of the Colorado Division of Vocational Rehabilitation (DVR). All individuals that work with SWAP will also need to meet the eligibility requirements for DVR.
Each individual will need to complete the required paperwork for DVR in order to be eligible for SWAP services. The individual will also have a DVR counselor assigned to them. SWAP will coordinate and assist during this process. For more information about DVR, please visit their website.
It is important to note that SWAP is not a job placement service. Our goal is to help individuals get ready for employment so they are able to have successful outcomes.
