Transition planning is a vital component of the IEP process for students, starting at 15 years of age. Jeffco offers a variety of transition services including the School to Work Alliance Program (SWAP) and Jeffco Transition Services (JTS).

Jeffco Transition Services

Jeffco Transition Services (JTS) is designed to support young adults with disabilities as they move from high school into adulthood. This program offers a variety of services and supports focused on helping students develop the skills and connections they need to be successful in post-secondary education, employment, and independent living.  

Who Does Jeffco Transition Services Serve?

Jeffco Transition Services serves students with disabilities who are typically between the ages of 18 and 21, who are still eligible for special education services under the Individuals with Disabilities Education Act (IDEA). These students are residents of Jefferson County, Colorado.  

How Does Jeffco Transition Services Serve Students?

Jeffco Transition Services serve students in a variety of ways, often through:

  • Individualized Planning: Developing personalized transition plans based on each student's unique needs, interests, and goals.  
  • Skill Development: Providing instruction and support in areas such as job readiness, independent living skills (like budgeting, cooking, and transportation), social skills, and self-advocacy.  
  • Community-Based Experiences: Offering opportunities for students to gain real-world experience through internships, volunteer work, and community exploration.  
  • Collaboration: Working closely with families, school staff, community agencies, and potential employers to ensure a coordinated and comprehensive approach to transition.  
  • Post-Secondary Exploration: Assisting students in exploring options for further education, vocational training, and career pathways.  
  • Connecting to Resources: Linking students and their families with relevant community resources and supports that can assist them after they exit the school system.
